Assess charts and patients to complete patient care plans, MDS, RAPS, and other required documentation to meet Federal and State requirements. Also sit in on care conferences and participate in IDT processes.
RN preferred, but an LPN may be considered with previous long term care/Skilled Nursing and Rehab setting.
Must have prior MDS experience, prefer MDS certified applicants.
Must have the ability to work on your own and complete tasks by strict deadlines. Must be able to work with other providers to complete needed assessments.
